Penile Enlargement

Beverly Hills board-certified plastic surgeon Dr. Glenn Vallecillos, MD, FACS, approaches aesthetic medicine as the intersection of anatomical precision and psychological well-being. While cosmetic surgery for men has grown dramatically, having increased 325% since 2000, penile enlargement remains one of the most personal and least openly discussed areas within this space. That is exactly why the environment at the practice of Dr. Vallecillos is discreet, judgment-free and open to any conversation about penile enlargement procedures you need to have.

Procedure Overview

Penile enlargement, also called phalloplasty or male enhancement, combines surgical and nonsurgical techniques to increase girth, flaccid length or a combination of the two. Men pursue these procedures for a range of personal reasons, and Dr. Vallecillos approaches each case with your dignity and sense of well-being at the forefront of his mind. His extensive background in minimally invasive soft-tissue augmentation and high-definition body contouring positions him to deliver proportional, natural-feeling results that complement your existing anatomy seamlessly.

Preparation and Consultation

Every treatment plan begins with a thorough private consultation. Dr. Vallecillos reviews your medical history, evaluates your anatomy and takes time to understand your goals before recommending any specific approach. Candidates for surgical enhancement may be asked to discontinue blood-thinning medications in the days prior to the procedure. Smokers are advised to stop smoking ahead of any surgical treatment to support proper healing.

Anesthesia

The anesthesia protocol depends on the technique being used. Injectable HA filler treatments are performed under local anesthesia in the office. Surgical procedures, such as autologous fat transfer and suspensory ligament release, are typically performed under general anesthesia or sedation.

Girth Enhancement With Hyaluronic Acid Filler

Dr. Vallecillos applies the same mastery behind his injectable treatments to penile girth enhancement using specialized hyaluronic acid (HA) dermal fillers. The filler is precisely placed along the penile shaft in-office, increasing circumference with immediate, visible results.

Girth Enhancement With Autologous Fat Transfer

For men seeking more lasting results, autologous fat transfer offers a permanent and natural-feeling solution. Fat is harvested from a donor site using advanced liposuction techniques. It is then purified and strategically grafted along the penile shaft. Because the tissue is your own, the augmented area integrates seamlessly and feels consistent with the surrounding anatomy.

Penile Lengthening via Suspensory Ligament Release

Surgical lengthening involves releasing the suspensory ligament, the fibrous band that attaches the base of the penis to the pubic bone. Carefully dividing this ligament allows a portion of the internal penile shaft to externalize, visibly increasing flaccid length. This procedure is performed under general anesthesia and is commonly combined with fat grafting to address both length and girth in a single session.

Benefits of Penile Enlargement

The Fifth International Consultation on Sexual Medicine (ICSM 2024) recognizes penile augmentation as a growing area of evidence-based practice, reflecting the increasing demand for safe, standardized approaches to male enhancement.

The benefits of these procedures are both physical and psychological:

Increased girth through controlled, precise augmentation using either HA filler or your own harvested fat

Greater flaccid length through surgical suspensory ligament release, a permanent anatomical change

Natural look and feel, particularly with autologous fat transfer, which integrates seamlessly with surrounding tissue

Scalable options, from a same-day nonsurgical injectable with no real downtime to a comprehensive surgical enhancement for more significant results

Am I a Good Candidate for Penile Enlargement?

Good candidates are adult men in overall good health who carry realistic expectations about what penile enlargement can achieve. The right approach can vary dramatically from one person to the next. Candidacy is determined during a one-on-one consultation with Dr. Vallecillos.

Most healthy adult men will qualify for the HA filler approach. Surgical enhancement via fat transfer requires sufficient donor fat and good skin integrity at the harvest site. Patients with chronic health conditions, those taking medications that affect bleeding or healing, and patients with prior surgeries in the pelvic region will have their eligibility discussed in detail during the consultation process.

How Penile Enlargement Is Customized to Each Patient

What works for one patient's anatomy and goals may be entirely wrong for another, which is why Dr. Vallecillos conducts a detailed physical evaluation before any treatment is recommended.

During your consultation, he assesses penile girth, existing flaccid length, skin quality, pubic fat distribution and overall body proportions. For fat transfer candidates, the quality and volume of available donor fat at the harvest site plays a direct role in procedure planning. For ligament release, the depth and attachment of the suspensory ligament shape the surgical technique.

Your preferences guide the plan as much as your anatomy does. Are you looking for modest refinement or a more significant change? Do you want to start with a nonsurgical option before committing to surgery? Is your primary concern girth, length or both?

As Dr. Vallecillos describes his practice philosophy: "We partner closely with our patients and strive to establish strong relationships with them."

Recovery and Aftercare

Recovery timelines differ depending on the technique used in your treatment approach.

HA filler injections require minimal recovery. Most patients resume daily activities the same day. Sexual activity should be avoided for a short period (Dr. Vallecillos will specify the exact timeframe) to allow the filler to settle evenly and achieve its final result.

Surgical enhancement (fat transfer or ligament release, or a combination) requires a more structured recovery. Most patients are ready to return to non-strenuous work within a few days to one week. Strenuous exercise, heavy lifting and sexual intercourse should be avoided for approximately four to six weeks to protect graft survival.

Dr. Vallecillos and his team provide comprehensive aftercare instructions and remain available throughout your recovery for any questions.

Second Level Heading

Is penile enlargement surgery safe?

When performed by a board-certified plastic surgeon with expertise in soft-tissue augmentation, penile enlargement carries a strong safety profile. However, as with any surgical procedure, risks still exist. These include infection, asymmetry and partial fat reabsorption in the case of fat transfer. Dr. Vallecillos will discuss all relevant risks in detail during your pre-operative consultation.

How much girth or length can I expect to gain?

Results vary based on the technique used, the volume of filler or fat transferred and your individual anatomy. Girth enhancement with HA filler or fat transfer can produce meaningful increases in circumference. Ligament release typically adds a visible increase in flaccid length for most patients. Dr. Vallecillos will establish realistic, individualized expectations during your consultation.

Will these procedures affect sexual function?

Performed correctly, none of the techniques offered by Dr. Vallecillos are intended to impair normal sexual function. Ligament release may alter the angle of erection slightly for some patients, a nuance that Dr. Vallecillos discusses openly ahead of surgery. A period of sexual abstinence following any of these procedures is required to support healing; the exact timeframe depends on the technique used.

How long do results last?

HA filler results are temporary, typically lasting 12 to 18 months, after which maintenance treatments can extend them. Autologous fat transfer results are longer-lasting, though some degree of fat reabsorption is expected during the first few months following surgery (additional grafting can be performed if desired). Suspensory ligament release produces a permanent anatomical change that does not reverse over time.

Is my consultation completely private?
