Calf Reduction
Disproportionately sized calves can impact the balance of the lower body and affect how clothes and boots fit. If you have struggled with large calves and are looking for a solution, surgical calf reduction may be the right fit. Dr. Glenn Vallecillos brings his experience as a board-certified plastic surgeon to the Beverly Hills area, where he has performed more than 10,000 plastic and reconstructive surgeries. Calf reduction preserves the natural lines of the lower leg while bringing the calves into proportion with the rest of the body.

What Is Calf Reduction Surgery?
Calf enlargement can result from a number of external and internal factors. In some patients, the gastrocnemius muscle is genetically large or has bulked from years of athletic activity. In others, extra volume in the lower leg is the result of localized fat deposits. Three different surgical techniques exist for reducing the size of the calves:
Partial Muscle Resection
โAn invasive procedure where a portion of the gastrocnemius muscle is removed.ย
Selective Neurectomy
โA method that focuses on cutting the nerves that stimulate gastrocnemius muscle growth.
Calf Liposuction
โA fat removal method that removes stubborn fat around the calf and ankle for a slimmer profile. This is the method that Dr. Vallecillos focuses on at his practice.
Am I a Candidate for Calf Reduction?ย
When you arrive at your consultation, Dr. Vallecillos will examine what is contributing to your lower legโs shape and size. If you have calf fullness from subcutaneous fat, you are a good candidate for calf liposuction. If your calves are predominantly muscular, Dr. Vallecillos can discuss this with you during your consultation.
In general, good candidates for calf liposuction:ย
- Are in good overall health with no uncontrolled medical conditions
- Have realistic expectations about what liposuction can and cannot achieve
- Maintain a stable weight close to their target body composition
- Have good skin elasticity in the lower legs, since liposuction does not tighten skin
- Are non-smokers or willing to stop smoking for at least 14 days before and after surgery
Your candidacy may be affected by the following:
- Significant skin laxity in the lower leg
- Circulatory or lymphatic conditions affecting the lower extremities
- Autoimmune conditions or a chronic illness that could impair healing
- A history of adverse reactions to anesthesia

How Is Calf Reduction Customized for Each Patient?ย
The distribution of fat (medial, lateral or circumferential), the quality and elasticity of the overlying skin, the relationship of the calf to the ankle and knee and your overall aesthetic goals all inform how Dr. Vallecillos plans and executes the procedure. If your goal is to reduce calf fullness primarily on the back of the leg, Dr. Vallecillos will use techniques that sculpt this area without disturbing the lateral contour.
Some patients are seeking more comprehensive refinement with changes around the circumference of the calf. The procedure will be carefully planned to preserve symmetry. Other nearby areas may also be treated, such as the ankles or thighs.
Procedure Walk-through
Pre-Operative Preparation
โDr. Vallecillos provides each patient with a pre-op checklist during the consultation process. This generally includes pausing certain medications and supplements that can increase bleeding risk and arranging transportation home after surgery.
Anesthesia
Local anesthesia combined with tumescent fluid is most common. Sedation or general anesthesia may be added depending on the scope of the procedure.
The Procedure
โA tumescent solution is infused into the treatment area. Tiny incisions, generally a few millimeters in length, are placed in inconspicuous locations near the ankle or behind the knee. Dr. Vallecillos uses a microcannula to remove fat.
Closure
โIncisions are closed, and a compression garment is applied immediately to support healing and shape.
Aftercare and Results Timeline
Lower legs bear more weight, so swelling tends to be pronounced and longer-lasting in this area. It is important to follow all of our instructions about compression garments to help speed up your recovery.
Here is what to expect from your healing timeline:
Week 1
โExpect significant swelling and bruising. Compression garments are worn continuously. Walking is encouraged in short intervals, and elevating the legs whenever possible helps minimize swelling.
Weeks 2 to 4
โSwelling begins to resolve noticeably. Most patients return to desk work within seven to ten days. Strenuous lower-body activity, prolonged standing and high-impact exercise should still be avoided.
Months 2 to 6
โThe final contour emerges as residual swelling clears. Most patients see their definitive result somewhere between three and six months post-op.
Procedures That Pair Well With Calf Reduction
Calf Botox
Some patients have a combination of fat and muscular bulk in the lower leg. In these cases, Dr. Vallecillos can pair calf liposuction with Botox injections into the gastrocnemius to slim the muscle alongside the fat reduction for a more comprehensive change in shape.
Thigh Lift
Patients who have lost a significant amount of weight or who have lax skin in the upper leg may benefit from a thigh lift to remove excess skin, while calf liposuction refines the lower portion of the leg.
Brazilian Butt Lift
Pairing calf liposuction with a BBL allows Dr. Vallecillos to refine the shape of the lower body. The fat harvested from the calves can be used to increase the volume of the buttocks.
